Initial Consultation Form for Psychotherapeutic Consultancy

The purpose of this initial consultation form for psychotherapeutic consultancy is for you to provide me, Jasmine Jarchevska, BA (Hons), Dip. Psych., UKCP Accred. (https://nursethesoul.com/about/), with information about yourself for beginning psychotherapeutic consultancy together.

Filling it out takes around half an hour, but you might want to spend longer. If it’s easier, you are welcome to write your answers into a Word or Pages document and send me a copy. I also have a printable version of the form if you would prefer to print it out, fill it out by hand and send me scans or photographs. Please just ask.

You are also not obliged to answer the questions listed below and if you prefer we can go the questions together when we speak. This may take over some of our session time to talk in more depth but we can get to certain questions at whatever pace feels right for you.

Of course everything you write is strictly confidential. When you press send at the end of filling out the form i will receive it straight to my email inbox. Once the psychotherapeutic consultancy begins your data will be retained for my reference purposes only. If psychotherapeutic consultancy cannot be offered your data will be securely destroyed unless otherwise discussed.

If you believe it is relevant to our psychotherapeutic consultancy sessions, please give any information on your early life (Mother’s pregnancy, birth, breastfed, childhood traumas, childhood illnesses, separations, abandonment, what family life was like, how parents and siblings were experienced, what were your favourite toys, did you have an imaginary friend, memories of being at school, early childhood friends, other important relationships ie nannies, teachers, who was there for you growing up).
