The purpose of this availability check form is for you to provide me, Jasmine Jarchevska, BA (Hons), Dip. Psych., UKCP Accred. (https://nursethesoul.com/about/), with some basic information before booking in an initial consultation.

This is because even though you are not required to continue after the initial consultation, it’s important to see whether our availability will match so that it would be possible to continue if you decided you wanted to.

Please note, at present I am currently only working on zoom.
